Sugar Isn't One Thing
“Added sugar” refers to sugars and syrups added during food processing or preparation — not the naturally occurring sugar in whole fruit or dairy, which comes packaged with fiber, water, protein, and micronutrients that change how the body processes it. This distinction matters because a lot of the fear around “sugar” broadly doesn't hold up when applied to an apple, but does hold up, with real dose-response evidence, when applied to the added and free sugars in sweetened beverages, desserts, and processed foods.
The Evidence
The clearest large-scale finding comes from a cohort study of over 30,000 U.S. adults tracked for up to 15 years (Yang et al., JAMA Internal Medicine, 2014). People who got 25% or more of their daily calories from added sugar had nearly three times the cardiovascular disease mortality risk of those getting under 10% — and the relationship held as a genuine dose-response gradient across the whole range studied, not just at the extremes. This is one of the more striking dose-response findings in nutrition epidemiology precisely because it moved in clean steps as intake rose.
On body weight specifically, a systematic review and meta-analysis of randomized trials and cohort studies (Te Morenga, Mallard & Mann, BMJ, 2013) found that among people eating freely (not on a fixed calorie prescription), increasing sugar intake was associated with weight gain and reducing it with weight loss — and the effect appeared to be mediated through total calorie intake, since sugar-sweetened beverages in particular don't trigger the same satiety response as solid, calorie-equivalent food.
On metabolic disease specifically, a meta-analysis of 11 studies and over 300,000 participants (Malik et al., Diabetes Care, 2010) found that drinking one to two sugar-sweetened beverages a day was associated with a 26% higher risk of type 2 diabetes and a 20% higher risk of metabolic syndrome compared to consuming fewer than one per month — one of the more direct links between a specific, common dietary pattern and a measurable disease outcome in the literature.
What Actually Matters in Practice
The World Health Organization recommends keeping free sugars under 10% of total calories, with additional benefit suggested below 5%. The strongest, most consistent signal in the evidence above comes specifically from sugar-sweetened beverages — liquid sugar that adds calories without meaningfully reducing appetite — rather than from moderate amounts of added sugar in an otherwise whole-food-based diet. This isn't a case for treating a small amount of added sugar in context as dangerous; it's a case for treating daily sugar-sweetened beverages and sugar-heavy processed food as the highest-leverage thing to cut, well before worrying about a teaspoon of sugar in your coffee.
